Age, 61 of Frankfort, passed away Monday, October 28th, 2019 at Silver Cross Hospital in New Lenox.

Jacklyn is survived by her loving family, husband, David F. Juby, children, David J. Juby, Jennifer (Jeff) Small, siblings, Sandra (Gene) Moseng, Mark (Dorothy) Hanicits, grandchildren, Madison Small, Logan Small, Cody Small and many nieces and nephews.

Jacklyn was preceded in death by her parents, Steve & Barbara Hanicits (Lizik).

Jacklyn was a member of St. Anthony Catholic Church in Frankfort. She was an animal lover and had a great passion for goats.

Family will receive friends at Kurtz Memorial Chapel, 65 Old Frankfort Way, Frankfort, IL 60423 on Friday, November 1st, 2019 from 3:00 PM to 8:00 PM. Funeral service, Saturday, November 2nd, 2019 with prayers at the funeral home at 1:00 PM and then to St. Anthony Catholic Church, 7659 West Sauk Trail, Frankfort, IL 60423 for a Mass of Christian Burial at 1:30 PM.
