50, of Mokena, passed away unexpectedly on Wednesday, December 23, 2015 at Silver Cross Hospital. Beloved husband of Robin (nee Johansen); loving father of Philip Duckworth; dear nephew of Phyllis and Leigh Yurgin; special cousin of Leigh (Michelle) Yurgin and Susie (Jeffrey) Popaeko; cherished uncle of Nathan and Caleigh Yurgin, and Alex and Jake Popaeko; loved by numerous in-laws. He was preceded in death by his parents Joseph and Geraldine (nee Jalasky) and his grandparents William and Helen Jalasky. Jeffrey was employed in marketing and sales for Follett School Solutions. In lieu of flowers, memorials in Jeffrey's name to the American Heart Association would be appreciated. Family will receive friends at Kurtz Memorial Chapel, 102 E. Francis Rd., New Lenox, IL 60451 for a Memorial Visitation on Wednesday, December 30, 2015 from 4:00PM until time of Memorial Service at 7:00PM. at which everyone is welcome.
